It is very important to report your card lost or stolen as soon as possible. You can go to http://www.myubcard.com and deactivate it online, or you can call 645-6344 during business hours. After business hours you can call 1-800-lost UB1 (567-8821). You may be held responsible for any use of the card until proper deactivation.
There is a $20 fee for replacing a lost UB Card.
If you find your UB Card after you have deactivated it, there is no charge to reactivate it as long as it has not been replaced. Once your lost card is replaced with a new one, the original card cannot be made active again.
