This form may be used by Students living off-campus or the campus apartments, faculty and staff. Residence Hall Students can get a Dining Plan from our customer service representatives in the UB Card Office. Please fill out the form below to initiate your UB Card Account. Your account will be opened within one business day and you will be notified by e-mail. For your convenience both Dining Dollars and Campus Cash accounts are opened. Once the account is opened you may log in to the Card Management System (CMS) and add to either your Dining Dollars or Campus Cash account or both, it is your choice. Please note that there is a 2 business day (non-weekend and non-holiday days) delay to be able to use the CMS after you submit this form.
